So sad, he couldn't tell death to f-off a second time. one of the very greats.

It was in fact that one of the Thrust-reversers deployed shortly after take-off. Totally a mechanical malfunction.

There was just a little bit more to that story in that Lauda had to challenge Boeing to get them to reveal the truth about what happened and to admit that control was unrecoverable after deployment of the thrust reverser.
